Sentara is currently hiring aCommunity Health EducatorPosition is located in Rockingham, VA.

Sentara RMH Medical Center began as Rockingham Memorial Hospital in 1912. Serving a seven-county area with a population of close to 218,000 residents, the 238-bed community hospital partnered with the Sentara Healthcare system in May of 2011.

Sentara is a 125-year-old, not-for-profit system that operates more than 100 sites of care throughout Virginia and North Carolina, including 12 acute care hospitals: seven in Hampton Roads, one in Northern Virginia, two in the Blue Ridge region, one in southern Virginia and one in northeast North Carolina. This role will engages in health promotion activities and implements best practice public health strategies and interventions to promote health and well-being. Develops and presents educational programs; educations patients/clients on disease self-management and health behavior change strategies; promotes disease prevention and early detection; provides guidance on available resources.

**This role will have clients that are new parents or newly pregnant below 20 years of age, must want to work with adolescent population.

Hours- M-F 8a-4:30p-- hours flexible to when the client can meet for the home visit.

Job Requirements: Associate's Level Degree - Experience in lieu of education: Yes Preferred: Related - 1-2 years of public health or social work related experience preferred. 2 years related experience required in lieu of Associates degree.

Active Drivers license and a clean driving record required

Bilingual strongly preferred
