Sentara Health is currently recruiting for aFull-Time day shift, Care Coordinator.

This position is hybrid-remote, 2 days weekly in-office for candidates in Charlottesville, VA or surrounding area.

As a key member of the Population Health Team, the Care Coordinator will contribute to results as defined by the principles of value-based care. The position will collaborate with Population Health RNs, providers and practice staff to engage in long-term management approaches and optimize treatment for patients with targeted conditions to achieve desired health outcomes. Responsibilities will include implementation of patient care plans with RN or Clinical Pharmacy Specialist supervision and to monitor patient progress.

Job Requirements:

*High School Diploma required

*Associate Level Degree preferred

*Patient Care experience 2 years required

*Direct Patient Education experience 5 years, in lieu of degree OR

*LPN or Medical Assistant Certification OR

*CPhT with PTCB Certification may be considered

*Population Health experience preferred

In lieu of the degree requirements stated above, five years direct patient education experience will be considered or LPN or Medical Assistant certification or CPhT with PTCB certification.
