Job Description

The Palliative Medicine & Comprehensive Care program at Inova Fairfax Medical Campus consists of an interdisciplinary team of 5 physicians, 2 advance practice providers, 2 social workers, a palliative RN, a hospice coordinator, and a chaplain. Our comprehensive, expert team, provides specialty consultation throughout the 923- bed hospital campus, with a strong focus on both advanced symptom management and serious illness communication.

Job Responsibilities:

  • Participating in regular IDT meetings to optimize clinical integration, efficiency, and effectiveness of service delivery
  • Triaging and assigning palliative consult requests
  • Responding to incoming phone inquiries for palliative care
  • Communicating effectively with all referral sources to ensure patients' needs are met
  • Counseling on advance care planning and completing ACP documentation
  • Participating in family meetings and goals of care conversations
  • Carrying out symptom assessment to aid in triaging of palliative resources
  • Providing information to patients and families about palliative care and ambulatory palliative resources
  • Conducting nursing and team member education about palliative and end of life care
  • Participating in and sometimes leading team quality improvement efforts
  • Reviewing transitions of care, including discharges, for accuracy
  • Serving as an ambassador for palliative care throughout the hospital campus


Job Requirements:

  • One year of registered nurse experience required, preferred experienced with palliative care service.
  • Solid Critical Care nursing experience is highly desired
  • BSN from an accredited school of nursing. If RN has an Associate Degree in Nursing (ADN); they must complete BSN within 24 months of their start date
  • RN license to practice in the Commonwealth of Virginia
  • A Basic Life Support (BLS) certification issued by the American Heart Association (AHA) is required
