Description

Position Summary

Sovah Health - Danville is looking for an Oncology Discharge Coordinator who collaborates with all members of the professional and nonprofessional staff to ensure an adult patient will leave Sovah Health Danville with all patient specific documents, appointments, medications and transportation, as needed, to continue their recovery in the community and to avoid the potential for readmission.

* Completes patient checkout procedures (Schedule tests, referrals, and follow up appointments).

* Answers incoming calls and assist customers any questions or needs.

* Maintains accuracy of data, scheduling, and follow up care in Oncology department computer software.

* Schedules follow up appointments and forward referrals to other physicians and ancillary services.

* Responds to incoming calls using appropriate telephone etiquette.

* Orders dietary supplies and office supplies in coordination with manager / director.

* Completes and maintain patient FMLA documents with combination with the physician.

* Scans documents, reports, and results from outside agencies into Oncology software.

* Obtains pre-certification for various procedures as a back up to the office coordinator.

* Works with physicians and management staff to ensure that the needs of our patients are met and reports concerns in a timely manner.

* Up-holds the standards of compliance for multiple governing bodies (i.e. Infection Control Standards, Center for Medicare & Medicaid Services, and The Joint Commission).

* Coordinates with insurance companies, community based agencies and providers, as needed, to ensure approval of necessary supplies or services.

Qualifications

Minimum Qualifications

Minimum Education

High school diploma or equivalent required. Associate's Degree in Medical Assisting / Assistant preferred

Required Skills

Two (2) years' experience in a medical office setting required.

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Requires flexibility in scheduling and ability to work all shift patterns including week-ends and holidays to insure continuity of care and appropriateness of patient access coverage to the patients of Sovah Health Danville.

Certifications

Certificate in Medical Assisting / Assistant preferred
