Job Summary:

This position must demonstrate a commitment of quality service to our patients, the public and our co-workers. This position is accountable to provide patient care responsibilities under the supervision of a Registered Nurse.

Essential Functions & Responsibilities:

  • Perform reassessments related to delegated interventions, not to include initial shift assessments.
  • Notify supervising RN of any change in patient condition in the course of providing delegated interventions at the time the change in patient condition is noted.
  • Provide reinforcement to the education given to the patient, family, and/or significant other, to include discharge instructions. May not provide initial patient education.
  • Administer medications as approved per MWHC policy and provide treatment interventions within the scope of practice as defined within the facility LPN CBOT as delegated by the supervising RN.
  • Document data collected, medications administered, and nursing care provided.
  • Collaborate with the RN in implementation, evaluation and modification of patient care plan based on collected data.
  • Collaborate with the health care team to participate in coordination of services and contribute to the continuous Performance Improvement process.
  • Maintain own Competency Notebook. Participates in educational programs and clinical experience to further enhance their nursing practice.
  • Interact effectively with patients and families to ensure quality care.
  • Follow policies and procedures of the specific unit and the Department of Nursing and supports the hospital and department mission, goals and objectives.
  • Maintain ongoing requirements for education and licensure.
  • Attend weekly IDT meetings as well as all staff, nurse, and other meetings as necessary.
  • Participate in on-call/holiday rotations per agency process and needs.
  • Conducts field nursing visits (as needed) to support case managers and nursing staff in both homes and facilities.
  • Monitors availability and referrals for Hospice services during week and weekends.
  • Ensures regulatory required clinical notification of unmade visits are faxed/sent to physician.
  • Maintain Hospice Aide schedules and collaborate with field clinicians and Patient Care Coordinator to ensure coverage.
  • Perform other duties as assigned.

Additional Duties for Support LPN, Palliative Care:

  • Conducts field nursing visits (as needed) to support case managers and nursing staff in both homes and facilities.
  • Provides clinical and administrative support to hospice and palliative care physicians and nurse practitioners as needed.
  • Monitors provider visit productivity and coordinates daily schedule for inpatient and community based services.
  • Reviews clinical and billing work queues to identify and correct necessary edits or other revisions to promote timely and accurate billing.
  • Assists in obtaining authorization of services from commercial and other payers as necessary.


Additional Duties for Support LPN, Home Health:

  • Monitors availability and referrals for Home Health Services. Maintains RN schedules and collaborates with Clinical Supervisor, Homecare Coordinators to ensure coverage.
  • Organizes, audits and completes daily, weekly, monthly, and quarterly reports to ensure KPIs for agency are monitored. Summarizes and communicates trends and variances to agency leadership when identified. Monitors and reports hospitalization of Home Health patients daily.
  • Ensures transfer and discharge summaries are sent to physicians in a timely manner. Ensures that all required clinical documentation required by the home health field clinician is complete and the encounter is closed to allow timely billing.
  • Collaborates with and provides prim ary clinical support to Director, Clinical Manager, and Clinical Supervisors. Facilitates communication of emergency situations in a timely and effective manner to maintain the highest level of patient safety. Establishes and maintains professional rapport with all internal and external stakeholders while communicating clearly, effectively and accurately.
  • Monitor availability and referrals for Home Health Services.
  • Assist Clinical Supervisor to ensure coverage and maintains RN schedules.
  • Review work queues in EMR and submit to Care Team for review and/or complete orders.
  • Ensure transfer and discharge summaries are sent to physicians, all required clinical documentation is complete and the encounter is closed to allow for billing.
  • Collaborate with and provides primary clinical support to Leadership.
  • Facilitate communication of emergency situations.

Qualifications:

  • LPN license in the State of Virginia.
  • AHA BLS Provider CPR required
  • Two years experience in nursing preferred.
  • Must meet the physical qualifications of the job.
