Sentara Healthcare is currently hiring a Clinical Pharmacy Specialist, Pharmacy Population Health!

This is aFull Timeposition with day shift hours and great benefits!

Position Status: Full-time, Day Shift

Standard Working Hours: 8:00AM to 5:00PM (ET). Hybrid work schedule. Will travel 2 x a week to the office in Charlottesville, VA.

Job Responsibilities:

  • Under the direction The Pharmacy Team Coordinator, Population Health, provides ambulatory pharmaceutical care services in collaboration with other health professionals. This may include performing various patient-focused clinical, educational, and informative functions in an ambulatory care setting as an interdisciplinary team member in support of patients, practitioners, and ancillary staff. Collaborates in the development and/or implementation of disease-state management pathways, medication initiatives and other formulary management activities.

    Activities include conducting patient outreach to complete comprehensive medication reviews and medication reconciliation, medication related quality gap closure, activities supporting transitions of care, provider consultation and other activities related to medication-related outcomes for our attributed populations, including SQCN and SACO (Commercial, Medicare, Medicare- Advantage, and Medicaid).

    May have the opportunity to serve as a primary preceptor for pharmacy students and residency programs. Works to promote high quality, evidence-based, cost-effective pharmaceutical care.

Required qualifications:

  • DLD - Doctorate Level Degree in Pharmacy

  • Minimum 3 years experience as a Pharmacist in a hospital, clinic setting or long term care

  • Completion of ASHP-accredited residency may be substituted for required experience.

  • Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Ambulatory Care Pharmacist (BCACP), or Board Certified Geriatric Pharmacist (BCGP) certification required within 3 years of hire.

  • Will consider pharmacists not licensed in Virginia due to relocation.

  • Must have Virginia license within 90 days of Sentara start date
